Osekhale’s journey

From behaviour consultation and respite as a teen to a confident move into adult residential support — one young man’s journey across Transitions.

Osekhale’s journey

Osekhale first came to Transitions through our Family Behaviour Consultant (FBC) program, where we built a strong connection with him and his family and got to know his strengths and interests. Soon after, he joined our Family Services program and was matched with dedicated respite workers.

Together, they shared meaningful activities, built routines and created opportunities for Osekhale to grow — while giving his mom the chance to be at work knowing he was safe and supported after school.

As Osekhale approached adulthood, his family faced a big transition: moving from FSCD to PDD funding and navigating the complex system of adult services. This can be overwhelming for many families, but Osekhale’s family didn’t have to do it alone. Our team walked alongside them every step of the way — helping with applications, exploring options and building a plan for the future.

Initially, it was expected that Osekhale would transition into our mobile support program. But through open conversations and thoughtful planning, his family made a decision that surprised and inspired everyone: with courage and trust, they chose for Osekhale to move into our residential program.

Today, Osekhale is stepping into an exciting new chapter. We are honoured to have supported him and his family through this important transition, and proud to continue being part of his journey.

His story is a powerful reminder of what’s possible when families are supported, relationships are built, and every step forward is taken together.
